The film is abnormal. There is increased density in the right mid zone.
Which lobe is the abnormality in?
It is not possible to place this in a particular lobe on the single projection. However, it is above the horizontal fissure meaning it cannot be in the middle lobe. Because of the wedge shape of the lower lobe, the opacity could be in the apical segment of the right lower lobe or in the right upper lobe.
How would you further manage the patient?
You could perform a lateral, but that wouldn't really take the diagnosis any further. Cross sectional imaging in the form of CT would be the next best test.
