This is a case of a patient diagnosed with thyroid-related orbital disease based on the radiological findings and thyroid profile. Thyroid orbitopathy should be differentiated from orbital pseudotumor, which affects the extraocular muscles without sparing their tendon insertions. The pattern of the affected muscles (inferior and medial rectus affected first) is also pathognomonic for thyroid-related orbital disorders.
