Complete radial tear of the medial meniscus

Case contributed by Dai Roberts
Diagnosis almost certain

Presentation

Locking and medial knee pain.

Patient Data

Age: 50 years
Gender: Male

Complete radial tear of the central aspect of the middle third of the medial meniscus. The central third of the medial meniscus is extruded and causes some bowing of the MCL.The extrusion indicates a loss of hoop stress. Areas of partial thickness articular surface loss in the adjacent medial compartment.

The cruciates are intact. Normal lateral compartment.

Areas of partial and full thickness articular cartilage loss in the patellofemoral joint, most severe in the lateral trochlea of the patella.

Case Discussion

Complete radial tears are also known as meniscal transections. They give rise to a ghost meniscus, and in this case, this is best appreciated on the coronal sequences.

Articular surface loss can be described in different ways on MRI, ranging from purely descriptive (areas of partial thickness loss) to more diagnostic statements (moderate osteoarthritis or moderate chondromalacia).
