Perilobular fibrosis isa pattern pulmonary fibrosis which is manifested by increased fibrosis along the peripheral aspects of a secondary pulmonary lobule (interlobular septa, visceral pleura and vessels) and can superficially resemblesresemble interlobular septal thickening5.

It is usually described with cryptogenic organising pneumonia (present in around 50-60 % of such cases).

On CT images it may be seen an arcade-like sign.
