The split pons sign describes the shape of the pons in patients with horizontal gaze palsy with progressive scoliosis (HGPPS).
The split pons sign may be appreciated on axial MRI sequences and describes a particular pattern of hypoplasia of the pons, whereby there is a deep midline dorsal cleft in the pons that extends ventrally from the fourth ventricular floor, seemingly partially 'splitting' the pons in two 1-3. This pattern of hypoplasia is characteristically described in horizontal gaze palsy with progressive scoliosis (HGPPS) 1-3.
