Vascular groove sign

The vascular groove sign is a radiological sign which is primarily seen on CT as radiolucent serpiginous or linear grooves extending from the periosteal surface of the cortex down to the tumour nidus referring to enlarged arterioles delivering blood from the periosteum to the hypervascular tumour nidus.

 Clinical importance

According to a study 1, this sign is moderately sensitive (~74-76%) yet seems to be highly specific (~90-96%) for distinguishing osteoid osteomas from other radiolucent bone lesions.

A correlating finding might be present in MRI imaging studies up to half of patients 3.
