A large osteochondral defect is present involving the articular surface of the medial femoral condyle with fragments seen within the joint.

Typical appearances of an osteochondral defect. In such cases, a careful search for the loose fragment should be undertaken, cognisant that there may be multiple fragments (as in this case).
