Sciatic nerve peripheral nerve sheath tumor

Presentation

Left buttock pain.

Patient Data

Age: 60 years
Gender: Male

Associated with the right sciatic nerve (tibial portion) there is a mass that is homogeneous low T1, heterogeneous high T2 signal, with heterogeneous postcontrast enhancement. It lies at the level of the ischial tuberosity.

Skin marker has been placed in the left subgluteal region, where there is a cutaneous lesion, which is low T1, high T2 with postcontrast enhancement.

Case Discussion

This is an interesting case of a peripheral nerve sheath tumor, schwannoma vs neurofibroma, affecting the sciatic nerve. On the T2 SPACE sequences especially, the mass is localized to the tibial portion of the sciatic with the peroneal portion not involved but bowed outwards over the mass.
