Presentation
Chronic weakening of the triceps surae, soft tissue edema above the calcaneus. No history of trauma.
Patient Data



There is marked thickening and increased signal of the distal part of the Achilles tendon, consistent with tendinopathy. Extensive edematous changes are visible around the tendon and in the Kager fat pad, in keeping with paratenonitis/peritendonitis. Features of retrocalcaneal bursitis are also present.
Incidental finding of os trigonum.
Case Discussion
The Achilles tendon lacks a typical tendon sheath, but instead, there is a connective tissue sheath, called paratenon, running around it. Therefore an inflammation around this tendon is called paratenonitis or peritendonitis.
