Acromioclavicular joint dislocation with coracoid avulsion of coracoclavicular ligament complex
Presentation
History of road traffic accident, pain in right shoulder.
Patient Data
Acromioclavicular joint dislocation with complete tear of superior and inferior acromioclavicular ligaments and superior displacement of clavicle relative to acromion.
Partial tears of trapezius muscle and tendon (clavicle attachment).
Bone fragments at the anterosuperior aspects of coracoid process. Complete tear of clavicular attachment of trapezoid ligament is seen. High-grade interstitial tears of conoid ligament are seen.
Hyperintensity and oedema is seen along the humeral attachment of inferior glenohumeral ligament.
Large SLAP tear is seen extending into anterior labrum, from 10 o’clock to 4 o’clock position.
Fracture of right 3rd rib is seen with marrow oedema and surrounding callus formation.
Note is made of a well-defined lesion in superior aspects of the spine of scapula - likely enchondroma (likely incidental finding).
Case Discussion
Above findings are suggestive of a type III injury of acromioclavicular joint (Rockwood classification) with coracoid avulsion of coracoclavicular ligament complex.
