Cholesterol granuloma

A cystic soft-tissue attenuation lesion at petrous apex causing its widening and some mass effect on adjacent temporal cortex. Coronal reconstruction (single image) shows the bony expansion.
Case Discussion
Although cholesterol granulomas are the most common cystic lesion of the petrous apex, they are usually associated with chronic otitis media, which is absent in this case. Still based on morphology, best radiological diagnosis would be cholesterol granuloma.
The patient went on to have a resection and histology confirmed cholesterol granuloma.
