Atraumatic onset of pain on the radial side of the right wrist for the last 4-5 weeks.

There is a thickening of the common extensor retinaculum involving abductor pollicis longus and extensor pollicis brevis tendons. There is anechoic fluid in the tendon sheath distal to thickening. Both tendons show a normal echo pattern without a tear. A vertical septum is not present between the two tendons. Local significant hypervascularity is present.
