L4-L5 central disc herniation with a detached disc fragment right posterosuperior to the disc herniation, compressing mainly the adjacent L4 nerve root. It appears isointense to the intervertebral disc on T1, slightly hyperintense on T2, and STIR with peripheral enhancement on post-contrast sequences, suggestive of a free disc fragment that has no continuity with the native L4-L5 disc.

L5-S1 disc degeneration with small disc herniation in the right subarticular zone.
