Presentation

Lower abdominal pain, nausea, and feeling unwell. Tenderness with guarding in the right iliac fossa, raised inflammatory markers, overweight. Assess for acute appendicitis.

An area of acute inflammation of fat with a thin hyperattenuating rim close to the redundant sigmoid loop is noted in the right iliac fossa and at midline.

Case Discussion

Epiploic appendagitis is a self-limiting ischaemic/inflammatory process due to torsion of a large and pedunculated epiploic appendage, or spontaneous thrombosis of the venous outflow resulting in ischaemia and necrosis.

Differential diagnoses include diverticulitis, appendicitis, and omental infarction.
