Obese patient with multiple medical comorbidities. For screening mammogram.
Patient Data












Findings: Bilateral large breasts (gigantomastia/macromastia) with normal fatty parenchymal density. Multiple scattered micro and macro-calcifications in both breasts; however, no suspicious soft tissue abnormality, architectural distortion, pleomorphic microcalcification clusters, abnormal skin thickening or nipple retraction is seen on either side. Multiple small sub centimeter lymph nodes with preserved fatty hila, likely reactive in nature, are seen in both axilla.
Impression: BI-RADS 2 mammogram (benign findings).
Case Discussion
- Obese patient (height=151 cm, weight=105.8 kg, BMI=46.40 Kg/m2) with history of diabetes mellitus, hypertension, obstructive sleep apnea and knee osteoarthritis.
- Based on the history, mammographic findings are suggestive of bilateral gigantomastia/macromastia (type 1a).
