Presentation

Several episodes of syncope over the past month and a history of posterior fossa tumor surgery.

Patient Data

Age: 70 years
Gender: Male

Well-defined left cerebellar cystic mass with CSF signal intensity, non-enhancing walls, mild surrounding edema, and a solid mural node with vivid enhancement and multiple internal flow voids abutting the pia mater adjacent to the sigmoid sinus. Note the overlying occipital bone defect and scalp scar.

Case Discussion

A case of recurrent hemangioblastoma with typical imaging features in a patient with a history of posterior fossa tumor surgery for the primary tumor.

Imaging features include an intra-axial cystic mass with an intense enhancing mural node with multiple internal flow voids and non-enhancing cyst walls surrounded by edema 1.

The treatment strategy consists of surgical resection preceded by preoperative embolization for large lesions and adjuvant radiotherapy for patients with incomplete resections. Recurrence rate reported is around 25% 2.
