Hypoxic brain injury - white cerebellum sign

Presentation

The patient had a GCS 3 upon presentation to A and E.

Patient Data

Age: 3 years
Gender: Female

Diffuse cerebral edema with loss of grey white matter differentiation.

Hyperdense cerebellum (white cerebellum sign/reversal sign).

The thalami are also relatively hyperdense compared to the cerebrum.

Case Discussion

Presence of the white cerebellum and reversal sign are compatible with the clinical diagnosis of a hypoxic brain injury.

Take note of the diffuse cerebral swelling as evidence by effacement of the sulci and cisterns, tightness of the ventricles and loss of grey white matter interface.
