History of breast implant placement eight years ago. Presenting with right breast pain.

Patient Data

Age: 40 years
Gender: Female

Ultrasound screening identifies a right breast implant in a retro-glandular position, with abundant echogenic material within the implant lumen, as well as between the intact fibrous capsule and the silicone envelope.

The ultrasound findings are consistent with an intracapsular breast implant rupture.

Case Discussion

Intracapsular breast implant rupture involves a break in the outer envelope of the implant with an intact fibrous capsule, so the leaked silicone gel remains contained within the fibrous capsule.

Common ultrasound findings include:

  • separation of the fibrous capsule and the silicone envelope with echogenic material between them

  • horizontal and parallel echogenic lines within the implant lumen (known as the "stepladder sign")

  • echogenic material or internal echoes within the implant lumen
