Adjacent to the Achilles tenon insertion an about 10 mm long circumscribed area of fluid signal is visible within the tendon, best appreciated on STIR. Subtle fluid accumulation in the retrocalcaneal bursa. Otherwise unremarkable study.

Considering the cliincal setting the imaging findings likely represent an intratendinous ganglion cyst, resulting from a prior longitudinal Achilles tendon injury.
