Symmetrical hyperintensities are seen along bilateral corticospinal tracts. Subtle hypointensities are seen in bilateral precentral gyri on GRE and T2 images.

Case Discussion

In the given clinical context, the MRI findings suggest possibility of motor neuron disease eg. amyotrophic lateral sclerosis. Slurring of speech is often an initial clinical symptom of the disease.
