Comminuted impacted interarticular fracture of the navicular.

Impacted fractures of the heads of the third and fourth metatarsals. Superimposition of the 5th metatarsal phalangeal joint suggests a dislocation. More suspicious looking on the lateral projection.

Comminuted intraarticular fracture of the navicular bone that is impacted by the head of the talus. There is lateral collapse secondary to comminution. Depression fracture of the medial-dorsal articular portion of the talar head. Impacted fractures of third and fourth metatarsal heads.
Dorsal dislocation of the fifth metatarsophalangeal joint.

Case Discussion

Classic case of multiple mid-foot and forefoot fractures leading to an overall unstable injury pattern. The medial angulation of the foot, combined with the impacted navicular fracture puts this patient at risk of a Chopart fracture dislocation.
The forefoot fractures are easily missed if one were to concentrate on the midfoot fractures.
