Presentation
Car accident, referred for severe positional upper cervical spine pain.
Patient Data

A lucent irregular line with slightly sclerotic margins in the base of the odontoid process that extends to the lateral mass of the C2.

CT images in bone window and axial, coronal, and sagittal planes depict a lucent irregular line with slightly sclerotic margins in the base of the odontoid process with extension to the lateral mass of the C2.
Case Discussion
This case demonstrates an odontoid process fracture that involved the base and extended to the lateral mass of C2, suggesting a type III Anderson and D'Alonzo classification. This type of odontoid fracture is relatively stable if not excessively displaced, and has the best prognosis for healing because of its larger surface area.
