Orbital roof and floor blowout fractures

Presentation

Mechanical fall onto concrete with significant swelling over the right eye with diplopia on right gaze.

Patient Data

Age: 60 years
Gender: Female

Right orbital floor fracture: Large comminuted, inferiorly displaced fracture of the right maxilla orbital surface with inferior prolapse of extraconal fat and the inferior rectus muscle into the right maxillary sinus. Dependent fluid in the right maxillary sinus.

Right orbital roof fracture: Small superiorly displaced fracture of right frontal bone orbital surface, with superior prolapse of periorbital fat into the right frontal sinus. The position of the superior rectus muscle is normal.

There is swelling of the right inferolateral orbital and right periorbital soft tissues. No intraorbital hemorrhage or exophthalmos. Globes are intact. Mandible and TM joints are unremarkable.
