Right-sided spinous process L3 bone exostosis with medullary continuity.

Case Discussion

Typical appearance of osteochondroma of vertebral spinous process. Next step should be MRI to assess cartilage cap thickness because a cartilage cap of over 1.5 cm in thickness is suspicious for malignant degeneration.
