Mid-leg localized pain aggravates at night and responds to analgesics.

A mid diaphyseal tibial tiny lytic cortical lesion is seen surrounded by extensive sclerosis and new bone formation and corresponds to the site of maximal pain. No cortical destruction or extra osseous soft tissue extension.

The case represents classical appearance of osteoid osteoma as a cortical lytic lesion surrounded by sclerosis and new bone.
