Presentation

Sharp pain during sexual intercourse followed by swelling of his penis.

Age: 45 years
Gender: Male

There is a 15 mm breach in the tunica albuginea of the right cavernous body giving rise to a large hematoma, that appears mostly hyperintense on both T1 and T2 weighted sequences.

Case Discussion

Penile "fracture" is actually a rupture of the wall of the corpus cavernosum caused by trauma to an erect penis, most commonly during sexual intercourse or autoerotic manipulation. MRI is helpful especially in the presence of massive swelling and hematoma.
