Unilateral left leg pain during workouts.

The left popliteal artery is thrombosed. The left popliteal artery is not displaced, but the medial head of gastrocnemius inserts more lateral than usual; the artery passes medial and beneath the muscle.
The left popliteal vein is not entrapped.
Right popliteal anatomy normal.
Case Discussion
MR angiography with and without plantar flexion is indeed of paramount importance. However, due to technical issues with the MR sequences on our console, proper diagnostic imaging quality was not achievable. In this case, however, the left popliteal artery is obviously thrombosed.
