Presentation

Slipped and fell onto left hand one day ago.

Patient Data

Age: 35 years
Gender: Male

Left elbow

x-ray

Undisplaced fracture neck of radius.

The posterior fat pad is not visualized and the anterior fat pad is parallel to humerus, excluding a joint effusion.

Case Discussion

Radial head/neck fractures are the most common elbow injuries in adults. Most are accompanied by a joint effusion, detected on a lateral radiograph as positive posterior fat pad and sail signs which are negative in this case.

The elbow joint capsule attaches to the radial neck, so radial neck fractures may be intra-synovial or extra-synovial and do not necessarily present with joint effusion.
