From the case:
Spiral tibial fracture with 90 degrees of rotation

Download
Info

Spiral fracture of the distal tibial shaft with 90 degrees of lateral angulation (so the lateral projection of the leg has the ankle appearing as a frontal projection).
Spiral proximal fibular shaft fracture with mild lateral displacement, in keeping with a Maisonneuve fracture.
Ankle syndesmosis is not widened.
Case Discussion
Uncommon to see such angulation in a fracture.
