Enlarged and tender thyroid. Hyperthyroid on serum biochemistry.

There is globally reduced uptake in the thyroid, which is just discernable above background soft tissue activity. Uptake appears mildly heterogenous, however there are no dominant hot or cold thyroid nodules demonstrated on the images. 

Case Discussion

Decreased uptake of tracer in a hyperthyroid patient is typical of subacute hyperthyroidism. There are several different causes, including subacute granulomatous thyroiditis (as was the case with this patient) or post-partum thyroiditis. The disease course is usually self limiting.
