Presentation
Acute right testicular pain for almost 6 hours. No history of trauma.
Patient Data

Right testis is enlarged (42.5 x 28.4 mm) with homogeneous echotexture, decreased flow on colour Doppler imaging. Right epididymis is also enlarged in size (15.4 x 9.2 mm) with an inhomogeneous appearance. The spermatic cord near the epididymis has the appearance of a whirlpool demonstrated on greyscale ultrasound, very little flow on Doppler imaging. Small right-sided anechoic hydrocele.
Left test is normal size (35 x 13 mm). The head of the epididymis has three cystic structures (the largest one 8.8 x 5.5 mm). Small anechoic hydrocele.
Case Discussion
The diagnosis of right testicular torsion was confirmed intraoperatively. The testis was viable and right orchidopexy was performed.
