Presentation
Loss of consciousness, head trauma.
Patient Data

There is a fracture in the left parietal bone.
A large biconvex heterogeneous (swirl sign) hyperdense epidural hematoma in the left temporoparietal region causing mass effect, midline shift and subfalcine herniation.
There are also brain contusions at the opposite site of impact (contrecoup) at the right frontal and temporal lobes with moderate perifocal edema.
Small volume sulcal subarachnoid hemorrhage at the opposite the site of impact (contrecoup).
Case Discussion
CT is the preferred imaging modality for evaluating patients with acute head injuries. It is fast, non-invasive, widely accessible, and has minimal contraindications. CT offers significant advantages in traumatic brain injury (TBI) assessment due to its high sensitivity in detecting acute intra-axial and extra-axial hemorrhages, mass effects, ventricular abnormalities, and bone fractures.
