Presentation
Posterior shoulder dislocation in a cycling accident 2 weeks before. Radiographs not available.
Patient Data

Triangular impression fracture of the anterior superior humeral head. Hemorrhage, probably rupture of the middle glenohumeral ligament. Postero-inferior labroligamentous lesion of the glenoid (reverse Bankart).
Case Discussion
The articular surface of the humeral head is involved in about half of these fractures following posterior shoulder dislocation, also known as reverse Hill-Sachs lesions.
Trough fracture is a linear compression fracture of the anteromedial aspect of humeral head as a result of posterior shoulder dislocation.
