Discussion:

Liver cirrhosis with a large infiltrative and extensive left liver lobe malignant hepatic focal lesion consistent with  infiltrative hepatocellular carcinoma (HCC), with malignant left portal vein thrombosis, as well as two right liver lobe smaller hepatocellular carcinomas, abdominal lymphadenopathy, ascites and splenomegaly.
