An elderly male presented with urinary complaints. The ultrasound shows a very small gallbladder with tiny calculi. The patient had an open cholecystectomy through a midline upper abdominal scar about 25 years back. There were no recurrent symptoms related to cholecystitis after the surgery.

The ultrasound findings favor gallbladder remnant with gallbladder calculi without cholecystitis.
