The carina crossover sign is a morphological sign that describes the right pulmonary artery crossing the carina midline anteriorly i.e. cranially to its expected course. It is a highly specific criterion for the diagnosis of pulmonary arterial hypertension (PAH) on axial CT/MRI and together with an enlarged pulmonary trunk results in 100% specificity and positive predictive value.
