A D-shaped left ventricle or flattening of the interventricular septum with a D-shaped left ventricular configuration is a feature described with significant right ventricular (RV) overload or right heart strain, such as that occurring with complications of a sizeable pulmonary embolus. It was initially described in SPECT and scintigraphic studies although similar appearances have also been described in echocardiography 1 and CT in similar situations.
