Perilobular fibrosis

Last revised by Liz Silverstone on 2 Mar 2025

Perilobular fibrosis is a pattern of pulmonary fibrosis along the peripheral septa of secondary pulmonary lobules and is a typical feature in some cases of organizing pneumonia 5 and eosinophilic pneumonia 7.

Pathology

Organizing pneumonia is a common response to lung injury and the histological correlate is polypoid fibroblastic aggregations plugging alveolar sacs, ducts and bronchioles, which typically accompanies other disease such as pneumonia, collagen vascular disease, hypersensitivity pneumonitis, medication-induced lung injury, aspiration pneumonitis, etc 6.

Radiographic features

CT

Perilobular fibrosis is seen as fuzzy peripheral arcades along the visceral pleura (arcade-like sign) or fuzzy curvilinear bands of increased attenuation 5.
