Perilobular fibrosis isa pattern of pulmonary fibrosis along the peripheral septa of secondary pulmonary lobules and is a typical feature in some cases of organising pneumonia 5 and eosinophilic pneumonia 7.

Pathology

Organising pneumonia is a common response to lung injury and the histological correlate is polypoid fibroblastic aggregations plugging alveolar sacs, ducts and bronchioles, which typically accompanies other injurious disease such as pneumonia, collagen vascular disease, hypersensitivity pneumonitis, medication-induced lung injury, aspiration pneumonitis, etc 6.

Perilobular fibrosis is seen as fuzzy peripheral arcades along the visceral pleura (arcade-like sign) or fuzzy curvilinear bands of increased attenuation 5.
