The left kidney shows two large cystic lesions showing a focus of calcification with multiple internal small daughter cysts measuring collectively 11.4 x 8.1 cm which seem to be communicating with each other with a possible communication to the calyceal system that shows urothelial wall thickening and internal echogenic component.

Unenhanced CT scan shows a large cystic lesion associated with thick wall, internal septations and higher internal density on left kidney suggesting hydatid cyst.
Case Discussion
Renal hydatid infection is extremely rare and is seen less than 5% of patients with hydatid disease.
