Diagnosis almost certain

Presentation

Small mass sensation over the 3rd metacarpophalangeal joint region palmar aspect of the left hand for one year. No trauma. Normal free finger movements.

There is a 4 x 7 mm fluid equivalent signal thin-walled cyst abutting the A1 pulley of the 3rd metacarpophalangeal joint.

There is no tendon thickening/tenosynovitis/flexor tendon tear.

Palmar plate is intact.

There is no metacarpophalangeal joint effusion.

MRI findings were suggestive of peri-A1 pulley ganglion cyst of the 3rd MCP joint on the palmar side.
