Thyroid volume with normal limits.
Heterogeneous thyroid parenchyma, mainly because of micronodules, with increased vascularity.
No thyroid nodule is identified.
Level VI (Delphian) lymph node present, morphologically normal.
Case Discussion
The patient's TSH was normal but thyroid peroxidase (TPO) antibodies were elevated, which in combination with the micronodular ultrasound appearance is very suggestive of Hashimoto thyroiditis, in which level VI lymph nodes are often prominent.
