Patient Data

Age: 50 years
Gender: Female
mri

Large well-defined sacral perineural cyst with spinal canal expansion from S2 to S4 level, eliciting a low signal on T1, high signal on T2 and STIR (CSF signals) with no enhancement on postcontrast sequences. Bony remodeling of the sacrum is noted from S2 to S4.

Anteverted retroflexed with multiple leiomyomas.

Case Discussion

Tarlov cysts or perineural cysts, are CSF-filled dilatations of the nerve root sheath at the dorsal root ganglion (posterior nerve root sheath).

On imaging, the diagnosis itself is usually evident and no alternatives should be entertained. Occasionally appearances are atypical and possible differential considerations include:

Complications are rarely described in the literature and include rupture of the cyst wall and hemorrhage within the cyst 1.
