Large right staghorn calculus, with ultrasound demonstrating dilated fluid filled spaces, without hydronephrosis. This is elegantly shown on CT with an enlarged right kidney, with a bear's paw appearance of xanthogranulomatous pyelonephritis (XPN) with a small perinephric collection.
