No fracture or malalignment. Well-corticated bony fragment lies superior to the talar neck and represents an accessory ossicle (os supratalare). Soft tissue swelling over the lateral malleolus.

An os supratalare is an accessory ossicle that projects superiorly to the head or neck of the talus. It can often be confused for an accessory navicular or os talotibiale or an avulsion fracture.
