The fat signal intensity lesion immediately anterior to the splenium of the corpus callosum is predominantly situated in the cistern of the velum interpositum. A thin linear element extends posteriorly and superiorly around the inferior aspect of the splenium of the corpus callosum. In fat saturated sequences its high signal is apparently completely eliminated.
Postcontrast studies were performed. No pathological contrast enhancement was detected. No congenital developmental anomaly seen. No vascular anomaly, such as azygos anterior cerebral artery, detected.
Conclusion:
Virtually all the abnormal tissue within the cistern of the velum interpositum has the signal characteristics of fat. It most likely is a lipoma, as dermoid would be expected to also contain non fat tissue elements.
