At the posteromedial aspect of the talar dome is a well-corticated bony fragment. This is consistent with an unstable osteochondral injury but appears chronic. Correlation with examination and prior imaging recommended. There is also spurring and subjacent small subchondral cysts at the anterior aspect of the tibial plafond. No significant soft tissue swelling. No acute fracture demonstrated.
