Lower back pain radiating to the buttocks and urinary incontinence.

Multiple average sized Tarlov cysts are seen at an upper level related to S2-3 exiting foramina on both sides. Another larger cyst is seen at a lower level causing significant remodeling of S3 vertebral body and displacement and compression to the cauda equina roots. No related bone marrow edema.
Case Discussion
Tarlov cysts are benign perineural cysts, typically occurring in the lower lumbar and sacral spine, usually seen at the existing foramina arising from exiting nerve roots. They are more common in females. These benign cysts are usually asymptomatic, however sometimes they can grow to a large size and cause pressure symptoms and bone remodeling as in this patient.
