History of infertility, presented to ER with pelvic pain.

Two uterine horns, two cervical canals and two vaginas are seen with retained fluid intensity with blood products noted within the left vaginal canal denoting hematocolpos, while the right uterine horn is within normal. There is also a right ovarian simple cyst.



Absent left kidney, with compensatory enlargement of the right kidney, which measures approximately 14 cm.
Case Discussion
Herlyn–Werner–Wunderlich syndrome is considered to be a rare type of Mullerian duct anomaly, it is characterized by uterus didelphys with blind hemivagina and associated ipsilateral renal agenesis.
Herlyn–Werner–Wunderlich syndrome is also known obstructed hemivagina, and ipsilateral renal anomaly (OHVIRA). It usually presents after menarche, with progressive pelvic pain during menses secondary to hematocolpos, so early diagnosis can help early treatment, ensure quick relief from symptoms and preserve the fertility and so prevent the complications.
