Right upper lobe veins drain directly into SVC, creating a left to right shunt.
Anomalous origin of right subvclavian artery. traversing behind esophagus as it crosses midline from left to right.

A partial anomalous pulmonary venous drainage due to emptying of right upper lobe veins into superior vena cava.
Case Discussion
PAPVD is due to drainage of pulmonary veins into SVC, IVC or coronary sinus, creating a left to right shunt, in this case, only right upper lobe demonstrating anomalous drainage. Because of relatively small size of the shunt, this patient was not symptomatic, and presented with unrelated symptoms.
